2010 Indiana Chapter Community Grants
1. Health and Hospital Organization/ Future Promises Program for Pregnant and Parenting Teens
Indianapolis, IN
In the fall of 2009, Future Promises Program moved into its fourth high school and is continually invited into subsequent high school in Marion Co. The need for consistent replication is important to ensure fidelity in the program activates that have shown, through evaluation, to be effective. In addition to funding specific program activities, this project offers additional outside program evaluation that leads to the development of a Future Promises Program Development and Implementation Manual.
2. Children First Center/ Children First Prenatal Program
Auburn, IN
Prenatal program for high risk expectant mothers in Northeast Indian. The target audience will be the uninsured those who have been involved with or exposed to drug use.
3. Vermillion Parke Community Health Center/ Healthy Mothers Healthy Babies
Clinton, IN
To provide and enhance risk reduction and services related to providing smoking cessation education for women of childbearing age for citizen of Parke and Vermillion County.
4. Wishard Foundation/ Prenatal Care Coordination Program
Indianapolis, IN
The primary objective of Wishard's Prenatal Care Coordination Program is to decrease the rate of infant mortality and the number of low birth weight infants by providing holistic health care to at-risk pregnant women in community setting.
5. Community Health & Wellness Center/WIC/ Healthy Beginnings
Bedford, IN
To provide smoking cessation services to pregnant women who come through our clinic for the WIC program and prenatal care. The would include increasing information and referral services to these pregnant women's well as offering the same services to their partners or other household members who smoke.

Centering Pregnancy Module Implementation
6. HealthNet, Inc./ Centering Pregnancy Program
Indianapolis, IN
To enhance the Centering Pregnancy Group Model of Care at all HealthNet OB sites and to become and approved Centering Pregnancy site through the Centering Healthcare Institute. The primary purpose is reducing preterm births amount HealthNet's high risk prenatal population.
7. Door/BMH Health Center/ Starting Out Right Together (SORT)
Muncie, IN
Starting Out Right Together will improve birth outcome in Delaware Co. by utilizing a Centering Pregnancy model of Care.
8. Union Hospital Maternal Health Clinic/ Centering Pregnancy and Baby Shower with Minority Health Coalition
Terre Haute, IN
To implement the Centering Pregnancy model of prenatal care in our prenatal clinic that serves primarily low income families in the Here Haute, Indiana area.
9. Family Health Services/ Mom Taking Care of Mom
Rushville, IN
This project is increasing access to services and expanded prevention education services to homeless youth with the purpose of reducing birth defects and improving the health of babies.
