2006 Massachusetts Community Grantees

Lynn Community Health Center

To provide outreach, prenatal education, support and case management for the growing population of African women and children who have come to Lynn through the Refugee program and through secondary migration.  The LCHC Somali case manager does outreach and case management with African patients with the goal of getting women into OB services in their first trimester and provide them with ongoing support.  Will enroll the families in MCH programs, such as: Medicaid, SCHIP, EIPP, WIC.  Aim to reach 50 individuals, Black.

 

Great Brook Valley Health Center

Develop a prenatal smoking cessation program in an attempt to improve their perinatal outcomes, by using the “5 A’s”.  The project provides smoking cessation education, counseling and support to women who smoke.  A prenatal smoking cessation educator/counselor trilingual in English/Spanish/Portuguese conducts in-person and phone sessions.  Aim to reach 200 Hispanic/Black/other minorities

Whittier Street Health Center

Launch Boston’s first Stork Nest.  This is prenatal education program offered to low-income, expectant Black women in the Roxbury neighborhood.  Aim to reach 50 low-income, expectant Black women, including women whose ethnicities are African, African-American, Caribbean, Cape Verdian and Latin American.  Implement a Whittier Stork’s Nest (WSN) project adopting March of Dimes Stork nest prenatal education program.  Operated in partnership with Boston area chapters of: Zeta Phi Beta Sorority; Pho Pi Zeta; and Delta Onicron Zeta.

 

Greater New Bedford Community Health Center

Enhance education and support services to high-risk Spanish speaking pregnant        women.  Provide a series of 5 prenatal classes and one postpartum class for Hispanic women 3 times a year.  They use the curriculum from Commenzando Bien.  A health care worker performs outreach activities within the Hispanic community including visits and dissemination of information to housing sites, churches and community centers.  Aim to reach 50-70 Hispanic women.  Provide assistance to these women in obtaining health insurance for their pregnancies, participating in an on-site WIC program, and receiving early and adequate prenatal care.
