Bois Forte Reservation Health & Human Services  $14,685
Maternal Child Health New Lives Program
The Maternal Child Health New Lives Program will enhance prenatal services and influence a decrease in infant mortality rates of the Native American population.  The goals of the program are to provide support and services to the Native American families of Bois Forte Reservation and surrounding areas throughout pregnancy and during the early years of a newborns life.

Duluth Clinic & St. Mary’s Birthplace    $18,000
Smoking Cessation: Success for Moms and babies
To provide smoking cessation services, education and support to pregnant women who smoke and educate patients regarding the benefits of not smoking and promote the usage of the “5A’s” method by healthcare providers to ensure referrals to QUITPLAN.

Minneapolis Department of Health and Family Support  $14,685
Healthy Sleep for a Healthy Start

Healthy Sleep for a Healthy Start will provide at-risk perinatal women living in Minneapolis and St. Paul neighborhoods, who are primarily African American and American Indian, with culturally appropriate safe infant sleep education and printed information following the American Academy of Pediatrics 2005 policy statement, providing them with the knowledge to adopt infant safe sleep practices. 

Olmsted Medical Center      $22,331
Healthy Start
Healthy Start, a comprehensive tobacco intervention program at Olmsted Medical Center (OMC) will increase smoking cessation services that are available to pregnant women.  The program will utilize staff specialized in tobacco intervention and motivational interviewing, that supports women during pregnancy and postpartum. 

Open Cities Health Center      $20,295
Community Health Connection Project
Open Cities Health Center proposes to reduce the infant mortality rate and poor birth outcomes among the African American population in St. Paul by having a culturally appropriate Community Health Worker / Doula who will work with pregnant women and adolescents during their pregnancies and for the crucial weeks after a baby is born.
