This project will allow EBCAP to utilize a well established and tested model to enhance their current scope of prenatal health services offered to their patients; improving overall health outcomes of mothers, babies, new families and all individuals across the life cycle.
Visiting Nurse Services of Newport & Bristol Counties - (2nd year). Young Parent Program Smoking Cessation Project. This program is a comprehensive home visiting model for teen parented families. The proposed project continues the smoking cessation initiative started in 2008.
Connecting for Children and Families, Woonsocket, RI - Parents As Teachers’ Preconception Risk Prevention. This program will integrate preconception risk prevention into CCF’s PAT home visiting program to support better access to quality health care & prevention services. |
