• How  To  Receive Up to $500 Free  Funding For Public  Health  Education  Materials

INSTRUCTIONS:

1. Review  the March of Dimes Product Catalog.  If you do not have a  catalog, it can be accessed online at http://www.marchofdimes.com/professionals/2222.asp .
2. Fill out the March of Dimes Product Order Form in the catalog, listing your selections, quantity needed, and price.
3. To request funding, write a cover letter that gives a brief description of your agency and how the requested materials will be used.  Please include in the letter the actual total amount requested from the catalog order form (maximum amount of $500.00, including shipping cost).  This amount should be equal to the total on the order form.
4. Complete and sign a  W-9 tax i.d. form for your agency.  Please note that these awards cannot be processed without this signed form.
5. Mail the letter, your order form, and the completed W9 form to  March of Dimes,  Attention:Jessica Mullen, Director of Program Services, 240 Stoneridge Drive, One Greystone Building, Suite 206, Columbia, SC  29210-8013

If approved, a check will be cut and mailed directly to your agency.  You will then order your materials directly from the March of Dimes Fulfillment Center.  Keep a copy of your order form for this purpose.  Upon receipt of items, please mail or fax a copy of the invoice to the March of Dimes office (fax number: 803-799-4549).

Please allow 4-6 weeks for this entire process to be completed.
