The top three qualified nominees from each category ill be invited to attend the 5th Annual Nurse of the Year Awards Celebration, a black-tie affair, on Saturday, November 7, 2009 at the prestigious Petroleum Club in Casper, Wyoming.  All nominees will be listed in the event's program and the winner in each category will be announced during the awards celebration.  In addition, each winner will receive a special award of recognition and will be included in the media announcements immediately following the celebration.

Completed nomination forms, including a write-up and resume, must be received at the March of Dimes chapter office no later than Friday, October 9, 2009.  Please return completed forms to the March of Dimes - Wyoming Chapter, 800 Werner Court #321, Casper, WY  82601.
